I give BOWEN Group the exclusive right to represent me for the following placement opportunity on the effective date listed below. Any other company which submits my name or resume for placements during this period has done so without my authorization and consent. By completing and submitting this form, I confirm that BOWEN Group has the exclusive right to seek placements for the below specified projects for me with the term of this authorization.

I may cancel this authorization at any time by notifying my assigned representative and resubmitting this form.
